目的探讨激光削除冷冻复苏囊胚1/3周透明带对临床结局的影响。方法回顾性分析2018年5月至2019年3月在我院生殖中心行冻融单囊胚移植的688个周期患者的临床资料,其中355个周期的囊胚在解冻移植前接受激光削除1/3周透明带辅助孵化(LAH组),剩余333个周期囊胚解冻后未进行透明带激光辅助孵化(NLAH组),比较两组患者的HCG阳性率、临床妊娠率、种植率、单卵双胎率及早期流产率。结果与NLAH组比较,LAH组的HCG阳性率(65.63%vs. 53.45%)、临床妊娠率(54.93%vs. 44.44%)和种植率(56.06%vs. 45.35%)均显著升高(P<0.05),而LAH组的单卵双胎率(2.05%vs. 2.03%)和早期流产率(6.67%vs. 6.08%)与NLAH组无显著性差异(P均>0.05)。结论冻存囊胚解冻后行激光削除1/3周透明带再移植可以提高妊娠率,可以作为临床上提高囊胚移植成功率的一个有效方法。
Abstract:Objective:To explore the effect of laser removal of one-third of zona pellucida on clinical outcomes in frozen-thawed single blastocyst transfer cycles.Methods: The clinical data of the patients including 688 frozen-thawed single blastocyst transfer cycles in our reproductive medicine center from May 2018 to March 2019 were retrospectively analyzed. Among them,the blastocysts of 355 cycles received laser removal of zona pellucid-assisted hatching before frozen-thawed transplantation(LAH group),and the blastocysts of the remaining 333 cycles were not subjected to zona pellucid-assisted hatching(NLAH group). The pregnancy outcomes including the HCG positive rate,clinical pregnancy rate,embryo implantation rate,monozygotic twins rate and early abortion rate were respectively analyzed between the two groups.Results:The rates of HCG positive rate(65.63% vs. 53.45%),clinical pregnancy(54.93% vs. 44.44%)and embryo implantation(56.06% vs. 45.35%)in the LAH group were significantly higher than those in the NLAH group(P<0.05). The monozygotic twins rate(2.05% vs. 2.03%)and the early abortion rate(6.67% vs. 6.08%)in the LAH group were not significantly different with those in the NLAH group(P>0.05).Conclusions: The treatment of laser removal of one-third of zona pellucida increases the clinical pregnancy rate in frozen-thawed single blastocyst transfer patients,which can be used as an effective method to improve the success rate of blastocyst transplantation in clinic.
