摘要:

自然流产是妊娠早期常见的并发症,其病因复杂,部分机制尚未明确。近年来,炎症反应在流产发生中的作用受到关注。多项研究发现,中性粒细胞-淋巴细胞比值(NLR)、血小板-淋巴细胞比值(PLR)、全身免疫炎症指数(SII)、全身炎症反应指数(SIRI)等血液炎症标志物因其检测简便、低成本、易于动态监测,成为评估流产风险的重要工具。本文系统梳理了NLR、PLR、SII、SIRI等新型血液炎症标志物在自然流产预测中的生物学机制、临床效能及争议,并提出基于动态监测的个体化干预框架,为优化妊娠丢失的风险评估体系提供新视角。

Abstract:

Spontaneous abortion, a common complication in early pregnancy, is associated with multifactorial etiologies and incompletely elucidated mechanisms. In recent years, the role of inflammatory responses in the pathogenesis of miscarriage has garnered significant attention. Accumulating evidence indicates that hematological inflammatory biomarkers, including the neutrophil-to-lymphocyte ratio(NLR),platelet-to-lymphocyte ratio(PLR),systemic immune-inflammation Index(SII),and systemic inflammation response index(SIRI),have emerged as pivotal tools for evaluating miscarriage risk due to their technical simplicity, cost-efficiency, and adaptability for longitudinal monitoring. This review systematically synthesizes the biological mechanisms, clinical efficacy, and controversies surrounding these novel biomarkers in predicting spontaneous abortion. Furthermore, it proposes a novel individualized intervention framework based on dynamic monitoring of inflammatory profiles, offering new perspectives for optimizing the risk assessment system for the pregnancy loss.
