目的探讨宫腔内人工授精(IUI)手术操作情况对夫精人工授精(AIH)临床妊娠率的影响。方法回顾性分析我院生殖医学科自2016年8月至2018年10月共546个AIH周期,根据术中操作情况分为置管顺利组(n=420)、精液返流组(n=45)、人工授精管壁血染组(n=62)、宫颈钳夹组(n=9)和使用探针并出血组(n=10),比较各组临床妊娠结局。结果置管顺利组临床妊娠率为15.2%,精液返流组、人工授精管壁血染组、宫颈钳夹组及使用探针并出血组的临床妊娠率分别为13.3%、14.5%、11.1%和0%;与置管顺利组相比,其余各组临床妊娠率有下降趋势,但各组间无统计学差异(P>0.05)。结论人工授精手术操作可能是影响人工授精临床妊娠率的关键步骤之一;手术过程中减少困难置管和出血的发生,有可能提高AIH的临床妊娠率。
