目的 探讨首次体外受精-胚胎移植(IVF-ET)患者的希望水平及其影响因素。方法 本研究是一项横断面调查。采用便利抽样法，对2023年2—8月在武汉大学人民医院生殖医学中心就诊的1 425例首次行IVF-ET治疗的患者进行问卷调查，调查问卷包括一般资料问卷、Herth希望量表(HHI)、抑郁-焦虑-压力自评量表(DASS-21)。分别采用Pearson相关分析和多元线性回归分析希望水平与负性情绪的相关性及希望水平的影响因素。结果 首次行IVF-ET患者希望水平总分为(35.68±3.09)分，积极态度维度均分为(2.88±0.26)分，积极行动维度均分为(3.11±0.36)分，亲密关系维度均分为(2.93±0.29)分。Pearson相关分析结果显示，希望水平总分与压力、焦虑、抑郁均呈负相关(r值分别为-0.135、-0.105、-0.189)(P<0.01)。多元线性回归分析结果显示，首次IVF-ET患者希望水平的主要影响因素包括焦虑、抑郁、个人月收入及饮食习惯(回归系数分别为0.063、-0.139、0.344、-0.314)(P<0.05)。结论 首次IVF-ET患者希望水平中等稍偏高，焦虑、抑郁、个人月收入及饮食习惯是其主要影响因素。
Abstract：Objective:To explore the level of hope and its influencing factors in patients undergoing their first IVF-ET.Methods:This study was a cross-sectional investigation that conducted a questionnaire survey on 1 425 patients who underwent their first IVF-ET treatment at the Reproductive Medicine Center of Renmin Hospital of Wuhan University from February to August 2023. The questionnaire included the one with general characteristics, the Herth Hope Index(HHI),and the Depression-Anxiety-Stress Self-rating Scale(DASS-21). Pearson's correlation analysis and multiple linear regression analysis were used to analyze the correlation between the hope level and negative emotions and the influencing factors of the hope level.Results:The score of hope was(35.68±3.09) totally, the score of attitude dimension was(2.88±0.26) on average, the score of positive action dimension was(3.11±0.36) on average, and the score of intimate relationship dimension was(2.93±0.29) on average. Pearson's correlation analysis showed that the total score of hope level was negatively correlated with stress, anxiety and depression(r=-0.135,-0.105,-0.189,respectively, P<0.01). The results of multiple linear regression analysis showed that the main influencing factors of hope level of patients undergoing their first IVF-ET included anxiety, depression, personal monthly income and dietary habits(regression coefficients were 0.063,-0.139,0.344 and-0.314,respectively, P<0.01).Conclusions:The hope level of patients undergoing their first IVF-ET is medium to high, and anxiety, depression, personal monthly income and dietary habits are main influencing factors.
