目的探讨近10年中国健康男性精液质量的变化情况。方法中华医学会生殖医学分会(CSRM)收集2008～2018年向中国七家精子库8 989位捐精者基本情况及精液参数,按照WHO《人类精液检查与处理实验室手册》标准分析精液体积、精子浓度、前向运动百分率和正常形态率等参数。资料分析方法采用ANOVA分析、偏相关分析、线性回归分析等。结果研究对象总样本量为8 989人,平均年龄为(26.26±5.35)岁。2008～2018年间,精液体积、精子浓度、精子总数、总活力百分率、前向运动精子百分率、精子正常形态率各参数均值有统计学差异(P<0.001);偏相关分析控制年龄、BMI因素,结果显示精液体积、精子总数、前向运动百分率、精子正常形态率均与时间呈负性相关,系数分别为-0.177、-0.135、-0.104、-0.084(P<0.001)。线性回归分析显示精液体积、精子总数、前向运动精子百分率、精子正常形态率变化与时间相关,均呈下降趋势(β=-0.148、-9.988、-0.134、-0.695;P<0.001)。结论 2008～2018年间,中国健康男性精液质量随时间变化呈下降趋势。
Abstract：Objective:To explore the changes of sperm quality in Chinese healthy men from 2008 to 2018. Methods:The basic information and semen parameters of 8 989 sperm donors from seven Chinese sperm banks from 2008 to 2018 were collected. The semen volume,sperm concentration,forward movement percentage and normal morphology rate were detected according to the WHO Laboratory Manual for the Examination and Processing of Human Semen. The data were analyzed with ANOVA,partial correlation analysis and linear regression analysis.Results:The total sample size of the study subjects was 8 989,and the mean age of the men was(26.26±5.35) years old. The semen volume,sperm concentration,total sperm count,total sperm motility,sperm forward movement percentage and normal morphology rate were statistically different from 2008 to 2018(P<0.001). The partial correlation analysis which age and BMI factors were controlled showed that the semen volume,total sperm counts,the sperm forward movement percentage,and sperm normal morphology rate were negatively correlated with time(β=-0.177,-0.135,-0.104,-0.084,all P<0.000). Linear regression analysis showed that the semen volume,total sperm counts,sperm forward movement percentage,and sperm normal morphology rate were declined with time(β=-0.148,-9.988,-0.134,-0.695,P<0.001).Conclusions:The semen quality of Chinese healthy men decreased with time from 2008 to 2018.
