控制性促排卵过程中血清黄体生成素、雌二醇与孕酮的相关性分析

目的分析控制性促排卵(COS)过程中黄体生成素(LH)、雌二醇(E2)、孕酮(P)值的变化趋势及相关性,探讨引起COS过程中血清P升高的相关因素,以便制定控制P升高的相应措施。方法选取在我院接受GnRH-a长方案治疗的175例患者,检测入组患者COS过程中注射Gn前1天、注射Gn第4天、注射Gn第67天和HCG日4个时间点的血清LH、E2、P值,分析LH、E2、P值的趋势变化;利用双变量相关分析法分析血清LH与P以及E2与P的相关性。结果 COS后期E2和P有同步升高的趋势;HCG日E2与P呈显著性正相关(r=0.404,P=0.000);HCG日LH与P呈显著性负相关(r=-0.287,P=0.000)。结论适当提高LH的水平可能会使P升高得到控制,而COS过程中控制Gn用量可能会通过血清E2水平缓慢升高从而控制P升高。

Abstract:

Objective: To investigate the changes in LH,E2,and progesterone levels during controlled ovarian stimulation(COS),and assess the correlation between serum LH & E2 level and progesterone level during different time points of COS in order to develop the corresponding measures to control the elevation of progesterone level during COS.Methods: A retrospective study was conducted in 175 patients undergone COS with the long protocol.The serum LH,E2,and progesterone levels were detected one day before gonadotropin(Gn)administration,the fourth day,sixth to seventh day after Gn injection,and on HCG day.The trend charts of the LH,E2 and progesterone levels during COS were plotted and analyzed.Bivariate correlation analysis was employed to investigate the correlation between serum LH & E2 levels and progesterone levels during different time points of COS.Results: Both E2 and progesterone levels exhibited an increasing trend in the late phase of COS.A significantly positive correlation was detected between E2 levels and progesterone levels on the HCG day(r=0.404,P=0.000),while a significantly negative correlation was observed between the LH level and progesterone level(r=!0.287,P=0.000).Conclusions: Elevated progesterone level can be controlled by properly increasing LH level.The elevation of progesterone level could be controlled by controlling usage of Gn though slowly increasing the serum E2 level.
