目的 探讨盆底肌电刺激联合生物反馈技术在治疗产后压力性尿失禁(SUI)中的临床疗效及应用价值。方法 选择2023年10月至2024年10月在成都市第五人民医院确诊为产后轻中度SUI的100例患者为研究对象,按照随机数字表法将纳入患者分为观察组(50例)和对照组(50例),对照组接受常规盆底肌训练(Kegel运动),观察组在常规盆底肌训练基础上联合盆底肌电刺激与生物反馈治疗。治疗周期为12周。比较两组患者的基础资料、治疗前后的国际尿失禁咨询问卷简表(I-CIQ-SF)评分、1 h尿垫试验漏尿量、盆底肌力评级、尿失禁生活质量问卷(I-QoL)评分,评估两组患者的临床疗效。结果 两组患者的年龄、体质量指数(BMI)、产次比较无显著性差异(P>0.05)。治疗结束后,两组患者的I-CIQ-SF评分、1 h尿垫试验漏尿量显著低于治疗前(P<0.05),盆底肌力评级及I-QoL评分显著高于治疗前(P<0.05);且治疗后观察组的I-CIQ-SF评分、1 h尿垫试验漏尿量显著低于对照组(P<0.05),盆底肌力评级及I-QoL评分显著优于对照组(P<0.05)。观察组的总有效率显著优于对照组(92.0%vs. 76.0%,P<0.05)。结论 盆底肌电刺激联合生物反馈技术能够改善产后轻中度SUI患者的症状,提高盆底肌力及生活质量,具有较好的临床应用价值。但未来研究仍需扩大样本量、完善研究设计、延长随访时间加以验证。
Abstract:Objectives:To explore the clinical efficacy and application value of electrical stimulation combined with biofeedback in the treatment of postpartum stress urinary incontinence(SUI).Methods:A total of 100 patients diagnosed as mild to moderate postpartum SUI at Chengdu Fifth People's Hospital from October 2023 to October 2024 were selected. According to the random number table method, they were divided into the observation group and the control group, with 50 cases in each group. Patients in the control group received conventional pelvic floor muscle training, while those in the observation group were additionally treated with pelvic floor electrical stimulation and biofeedback. The treatment lasted for 12 weeks. The baseline characteristics, International Consultation on Incontinence Questionnaire-Short Form(I-CIQ-SF) score, 1-hour pad test urine leakage, pelvic floor muscle strength rating, Incontinence Quality of Life(I-QoL) score before and after treatment between the two groups were compared, and the clinical efficacy of the two groups were evaluated.Results:There were no significant differences in age, body mass index(BMI) and parity between the two groups(P>0.05). After treatment, the I-CIQ-SF score and 1-hour pad test urine leakage in both groups were significantly lower than those before treatment(P<0.05),while the pelvic floor muscle strength rating and I-QoL score were significantly higher than those before treatment(P<0.05). Additionally, the observation group had significantly lower I-CIQ-SF scores and 1-hour pad test urine leakage after treatment than those in the control group(P<0.05),while the pelvic floor muscle strength rating and I-QoL score were significantly better than those of the control group(P<0.05). The total effective rate of the observation group was significantly better than that of the control group(92.0% vs 76.0%,P<0.05).Conclusions:Pelvic floor electrical stimulation combined with biofeedback significantly improves symptoms, pelvic floor muscle strength, and quality of life in postpartum SUI patients, providing high clinical application value. However, future research still needs to expand sample size, improve the study design, and extend follow-up time for verification.
